"Problem Reading Disc" Error

I don't understand this error message when playback is from the server (playback pauses and this message appears in the top right corner of the display). Is the error related to reading data off the HDD, or is it related to the Vault verifying that disc in the slot?
 
The error is hard drive related, not disc verification related. I've had these before from time-to-time, but they usually resolve on their own (meaning I can later play the movie without any problem).

I guess it's possible it could also be a sign that a disk is having issues (meaning a drive may be experiencing read errors, and this could be the start down the road to a drive failure).
